Like many nonprofits, the Alliance of Community Health Plans (ACHP) has a big mission and limited staff. One of their main goals for their online community and member experience platform is to improve efficiency and save staff time. When they launched their Higher Logic Community in 2018, itdrastically improved the efficiency of one of their biggest member benefits – member queries - by putting members directly in touch with each other. Community empowers their members to ask questions, share their experiences, and make connections without the ACHP staff serving as the go-between.Their recent upgrade to Higher Logic Thrive Platform has opened up even more opportunities to reduce staff burden and create an engaging member experience. The modern community design has refreshed members' interest with a 27% increase in first-time logins. Manager of Membership, Miriam "Molly" Edelman shared that“Since launching [Higher Logic Thrive Platform], we’ve had an increase in activity of our members. They’re posting a lot more, and more importantly they’re replying a lot more. We have a lot fewer unanswered questions in the portal.”And this is just the beginning...We can't wait to see how ACHP continues to thrive. ⚠️Webinar: Asset-based and Inclusive: how can community-orientated approaches embrace diversity⚠️The importance to wellbeing of being connected to family, friends and communities is now embedded within integrated care thinking and policies. Approaches such as asset-based community development, social prescribing, shared lives schemes, and community navigation are helping to transform health and care systems from being deficit focused to building on the aspirations and strengths of individuals and communities. There is, however, a risk that such approaches replicate inequalities in society through not being accessible to disadvantaged groups and working with the communities which already have the most assets. This webinar will consider how asset-based approached to integrated care can be aware of potential inequalities and pro-actively embed ways of working to address these in their practice and will include contribution from representatives from leading asset-based organisations and opportunity for discussion and debate.Speakers include Professor Robin Miller of IMPACT and the International Journal of Integrated Care (IJIC) and the University of Birmingham, Sarah McLaughlin of IMPACT and Older Person’s charity Mid and East Antrim Agewell Partnership and Ewan King of IMPACT and Shared Lives Plus.📆Tuesday, 5th December 2023⏰14:30 - 16:00 (IST/ BST)🔗https://lnkd.in/guK4h6Jy

Quality housing is critical to address the social determinants of health, and within the communities of color most affected by health and housing disparities, these systemic issues directly correlate to structural gaps in education, employment, public safety, adjudication, juvenile justice, environmental equity, community continuity, and overall quality of life. This is a much-needed start... Edwith Theogene

Moving Community Health Plan of Washington (CHPW) forward in our Diversity, Equity and Inclusion (DEI) work is one of our fundamental goals and supports our commitment to advance health equity and whole-person care. We cannot create and strive for an equitable and healthy society without integrating anti-racist practices within our internal and external work.We value our partners in our work serving Washington’s communities, so we’re pleased to share some highlights of our journey in 2023.Partner EngagementWe seek to reduce upstream causes of health inequities and remove unjust barriers to health and well-being by leveraging our deep community roots and catalyzing change through local and state-wide cross-sector partnerships.• Social Drivers of Health (SDOH) Strategy: In 2021, CHPW created a new SDOH strategy, grounded in three focus areas: housing insecurity, opportunities to support families and school-aged children, and data collection and analytics.-In 2023, CHPW focused on developing a Partnership Assessment Tool to identify key community-based organizations and non-traditional health partners to help advance this work.• Tribal Partnerships: Building relationships with Tribal nations plays a pivotal role in ensuring culturally-appropriate and responsive care. Our Tribal Affairs Administrator created educational sessions for non-Tribal providers and staff as well as resources including a database on telehealth and broadband services available for 29 Washington State and bordering Tribes.• Member Advisory Councils: CHPW has developed Member Advisory Councils (MACs) to help us connect directly with our members on what matters most to them, what works well and what is challenging for them as they engage with CHPW. MACs are regionally based to ensure representation of the communities we serve, and in 2023 we launched MACs in King County and Greater Columbia, with a goal to expand these councils across the state over time.• Community-Based Organizations (CBOs): Recognizing CBOs are delivering critical health-related services to Washingtonians, particularly those in historically marginalized communities, CHPW awarded $250,000 in Advancing Equity Fund grants again this year to 25 CBOs serving these key populations. The Advancing Equity Fund has provided a total of $980,000 to 68 community-based and Tribal-serving organizations across Washington State since 2020.#MyCHPW #PowerOfCommunity

Our institutions are not designed to work in community, but we are. These public and private funding opportunities share a concern for equity, though they may have a particular interest in any one of REACH’s focus areas: education; employment; food access/security; housing; or population health. https://lnkd.in/eq_PvdFy#community #sdoh #fundingopportunities #healthequity #socialdeterminantsofhealth
